Ensuring seamless transition of patients from hospital to home, Pupae provides comprehensive rehabilitation services. Our dedicated team of Nurses, therapists, carers, and support staff work together to deliver personalized care and support. With a holistic approach that includes physical, emotional, and nutritional support, we aim to enhance the well-being of our community members.
Our Promise
The Pupae hospital discharge program offers 25 hours of free carers support service that is supported by our lovely generous donors and sponsors, so you don't have to wait for a plan approval before you go home. Please enquire below for our eligibility criteria

Pupae Implements and support the use of structured clinical handover processes as described by the Australia Commission organisations to have clinical communication processes at transitions of care, across all levels of the organisation.
Transitions of care occur when all or part of a patient’s care is transferred between healthcare locations, clinicians, or different levels of care within the same location.
The Pupae Transition to home program adopts the quality standards and national health guidelines of the Department of health, and cater for patient who are funded under Transport Accident Commission TAC, National disability insurance scheme NDIS, National Injury insurance Scheme NIIS, Workcover, Transitional care, aged care home care Packages
The Pupae program was first trialled in 2022 with consented patient discharging from Sunshine hospital and Peter Macaulay cancer centre and was very successful in improving the following areas of care

community health care information, There is a change in clinician

A patient is transferred to another health service organisation (for example, from hospital to an aged care home, another hospital, community nursing or a palliative care service)

A patient is moved within an organisation (for example, to the general ward after surgery)

A patient’s care is discussed during multidisciplinary team rounds

A patient is transferred for a test or appointment

A patient is discharged home.

Allied Health Team
Pupae team has a teamed up with Allied health team specific organisation to provide holistic allied health services to ensure continuous rehabilitation at home.
Discharge destinations & Accommodation
We Liaise directly with department of housing, assist with Supported disability accommodation (SDA), Supported independent living (SIL Homes) supported residential services (SRS) Dementia care
Mental health
Mental health trained staff and specialist including psychologist, behavioral management and mental health support.
